Please show up 15 minutes early for your scheduled appointment time with your Military/Dependent ID card and any appropriate documents (immunization records for physicals, etc). This time allows us to properly screen you and address your concerns prior to seeing your provider. Failure to show up on time may result in the rescheduling of your appointment.
