Location of your DEERS/ID Card Appointment
Please note that the 2 DEERS/ID Card Site locations on Travis AFB have merged. They are both now located in Customer Central at 540 Airlift Dr. Building 381 Wing D-100, Travis AFB CA 94535.
Directions to our Site
From the Travis Main gate: Go straight on Travis Ave continue up the hill and then follow the signs for Customer Support/ID Cards. We're on the first floor of Building 381 D Wing.
ID Card Priority-Based
As of 6 October 2015, the TFSS MPF ID Cards Section is now priority-based. 1. Appointments 2. CAC Pin Resets 3. Active Duty in Uniform with CAC issues 4. All other CACs 5. All Others ID card/DEERs customers
A priority system will reduce excessive wait time and minimize mission impact/delay for specified priority customers.
Identification
ALL customers who are age 18 and older must have two (unexpired) forms of ID to receive a new ID card. One must be a state/government issued photo ID.
Adding a spouse to DEERS
When adding a spouse to DEERS, the sponsor needs to bring the following documents for their spouse: Marriage Certificate, birth certificate, social security card, & a photo ID. All documents must be originals or certified copies.
Dependent ID Cards
The sponsor must be present to sign the DD Form 1172. If not available, the dependent must have a previously signed 1172 (within 90 days of signature) by their sponsor and the VO/SSM or a Power of Attorney. Call for further information.
Adding children to DEERS
When adding a child to DEERS, the sponsor must have the following documents for the child: Birth Certificate & Social Security Card. If a stepchild is being added, the Marriage Certificate is also needed. Documents must be originals or certified copies.
Children Born out of Wedlock
Female Sponsors: Birth Certificate and Social Security Card; Male Sponsors: Voluntary Statement of Paternity/Court Order Establishing Paternity, Birth Certificate, SSN Card
Dual Military
If you are an Active Duty member and have married another Active Duty member, you will need to provide your Marriage Certificate, Social Security card and another form of ID.
Emergency Walk-in Customers
All emergency walk-in customers are seen on a first come, first serve basis. Wait times for walk-ins can be up to 2 hours depending on the day. Emergency walk-in examples are lost/stolen IDs, military/DOD civilian/DOD contractor work stoppage issues.
Full Time College Students
Students ages 21-23 must bring a letter from the school registrar certifying enrollment in a full-time course of study leading to an associate degree or higher, and anticipated graduation date.
2 valid forms of Identification as defined in the List of Acceptable Ddocuments I-9 form which is issued by the Department of Homeland Security. Initial college student IDs must be issued on or after the 21st birthday, not before.
Blocked out or Locked out
You will need to unlock your CAC card. Please have a 6-8 digit PIN number (you choose) ready when you come in.
Lost/Stolen CAC Card
Active Duty: assigned Air Force- memorandum signed by 1st Shirt for E1-E4 and two forms of ID. All others must have two forms of ID.
Lost/Stolen-Retired & Dependent
Anyone over the age or 21 will need to provide two forms of ID. College Students will also need to provide a letter from the Registrar's office.
Dependent Parent
You will need to provide a DFAS letter of approval, Service members Birth Ceritificate, valid Photo ID & Social Security card. Please note that all documents must be translated into English and certified.
Dependent Parent-In-Law
You will need to provide a DFAS letter of approval, SPOUSE's Birth Ceritificate, member and spouse's marriage certificate (if spouse is not in DEERS), a valid Photo ID & Social Security Card. Please note that all documents must be translated into english.
Dependent Ward
You will need to provide a DFAS letter of approval, Court Document placing the child in the members household for 12 mos, Birth Certificate and Social Security card. Please note that all documents must be translated into English and certified.
Contractors deploying overseas ID
Bring in two forms of ID - current, issued by state, federal or local government; SOFA agreement; SPOT LOA; DD Form 1172-2 requesting the overseas card. Forms must be properly filled out or they may be rejected.
Power of Attorney
If your Military Sponsor is unable to accompany you to your visit, a valid "General" Power of Attorney will allow you to conduct business on your sponsor's behalf. You may get this document at Legal.
Foreign National
If your spouse is a foreign national – marriage certificate, valid passport and birth certificate translated in english.
Civilian Employees
If this will be your first CAC card issue with a new position. You will need to provide 2 forms of identification such as a Driver’s License, Social Security Card, Birth Certificate.
Appointments
Please keep a copy of your appointment confirmation page. If you are running late, please call. If more than five minutes late for your appointment you will need to reschedule.

Submitted online depend ID request on 23 Mar 18, for a stolen ID. on 5 Apr, inquired the status. The person on the phone stated she look into it & call back. No call. Tonight I was out of town & my spouse had to be rushed to the DGMC ER but was delayed because she & driver had to be processed through the vistor center after the arrival of my son with his retired Military ID card. My spouse is on an overnight observation for risk of a heart attack. Why the delay of Depend ID cards?

July visit disappointing. Spouse's ID lost and tried to get replacement. It seem an expired ID card is not a valid document to issue new one. notwithstanding photo, DOB, ID number or SSN,, on it. Her Drivers License expired years ago. passport expired. Can't refill prescriptions. Have a DMV ID for her, but no second photo ID to use. A new spouse only needs one photo ID, a 40 year spouse MUST have 2 PHOTO IDs ??? How inflexible and dispassionate is that? Retired ID 1126116361
