EXPIRED ID'S
All expired ID's must proceed through the Moline gate (ID will be taken). Age 18/older, will still need to provide two(2) Current forms of ID's to be issued a new ID. One(1)State or Federal photo ID, the other has to be one(1)State/Federal ID or Document

PIN RESETS
CAC Pin Reset is handled on a walk-in basis only at the ID office. No appointments are needed for CAC Pin Resets. Blocked out or locked out you will need to unlock your CAC card. Please have a 6-8 digit PIN number (You Choose) ready when you come in.
CIV RETIREES/CHIPLESS CAC
To obtain a Civilian Retiree Chipless CAC, the Retiree must present an SF50 along with two(2) Current forms of State or Federal IDs (1 with photo).
SOLDIER, NEW RECRUIT/RENEWALS
Must be clean shaven, No jewelry, and wear the top portion of your uniform. Long hair must be pulled back and up off the collar. Two(2) current forms of State or Federal IDs (1 with photo) are required.
NEWLY HIRED CIVILIANS
All DOD Civilian Employees are requested to wait a minimum of 3 days from start date to allow the Human Resources Office time to create records. Two(2)current forms of State or Federal IDs(1 with photo) are required along with a Gov't E-Mail Address.
ROCK ISLAND ARSENAL EMPLOYEES AND CONTRACTORS
To obtain a CAC, you are required to have an AKO account prior to scheduling an appointment. Please contact your supervisor or COR/TA for assistance if you do not have an AKO account
CANCEL/RESCHEDULE AN APPOINTMENT
Customers will need to know their confirmation number to cancel/reschedule an appointment. Please keep a copy of your appointment confirmation page. If more than five minutes late for your appointment, you will need to reschedule.
DEPENDENT ID CARDS
For dependent ID cards, the sponsor must be present to sign the DD Form 1172. If the sponsor is not available, the dependent must have a DD Form 1172 signed (within 90 days of the appointment) by their sponsor and the VO/SSM or a POA
ADDING A SPOUSE TO DEERS
Sponsor brings following documents: marriage & birth certificates, social security card, and photo ID. If previously married, divorce decree is required. All documents must be originals or certified copies. English translations must accompany Foreign docs
ADDING CHILDREN TO DEERS
Sponsor brings the following documents: birth certificate & social security card. If a stepchild is being added, the marriage certificate is needed. Documents must be originals or certified copies. English tranlations must accompany all Foreign documents
FULL TIME STUDENTS AGE 21:
All College students must bring a letter from their school registrar's office stating that they are enrolled full time in an accredited college and anticipated graduation date. **College ID's are NOT an acceptable form of ID.**
CONTRACTOR NEW HIRE/RENEWALS
To obtain a CAC, the individual must be entered into the Trusted Assoc Sponsorship System (TASS).
MILITARY RETIREES
Must bring in Retirement Orders, DD214. Must present two (2) Current forms of ID to be issued a new ID card. Both of them must be State or Federal Government issued IDs (1 with photo). This applies to all customers!
Military Retiree and Spouse, Age 65
1 month before you have reached age 65, you should receive your Medicare card. Bring in Medicare card and must present two (2) Current forms of ID to be issued a new ID card. Both of them must be State or Federal Government issued IDs (1 with photo).
