Directions to Customer Services
From Main Gate: Take the first left after entering the main gate, then take the second right into the Club parking lot. Building 416 is to the right of the Club as you turn into the parking lot.
Scheduling Appointments
A scheduled appointment takes about 20 minutes per session. A session may consist of one CAC ID issuance or up to two Dependent ID cards. Please schedule multiple appointments based on the number and type of ID cards required.
Cancelling/Changing Appointments
If you need to cancel or change an appointment, please do so online or simply ring us on 01423 777 867/197/198 as soon as possible.
Walk-In Customers
Walk-in customers or emergency cases requiring same-day service are welcome but please be aware that you may have to wait. Scheduled appointments have priority over walk-in customers, so please make an appointment to avoid any inconvenience.

CAC Reset/Blocked Pin/Certificate Update
No appointment is needed for a CAC reset/blocked pin/certificate update. This corrective action will be conducted as a Walk-In service.
Lost or Stolen CAC/ID Cards
If an ID card/CAC has been lost or stolen, report the loss/theft to the base police, security office, and Customer Services. Customer Services will require a completed and signed Lost Form before a new ID can be issued.
Lost Form

Power of Attorney
If a sponsor is DEPLOYED or on TDY orders that prevent their presence for an appointment,a valid "General" or "Special" Power of Attorney will allow you to conduct business on your sponsor's behalf.
Marriage Notification/Spouse Enrollment
When adding a spouse to DEERS, the sponsor must be present and the enrollee must provide a valid photo ID, the original marriage certificate, and the Social Security card or tax document.
Spouse Enrollment - Foreign Nationals
If the spouse is a foreign national, submit the marriage certificate, 2 forms of photo ID, and birth certificate. All documents must be original or certified copies and translated into English.
Dependent ID Cards Part 1
To obtain a dependent ID card, the sponsor must be present or arrange to have a DD 1172 signed, witnessed by a Verifying Official or Notary Public, and on file in advance of the ID card issuance.
Dependent ID Cards Part 2
All dependents must surrender the existing/expiring ID card before a renewal ID will be issued. Additionally, dependents aged 18+ must provide a second form of valid photo identification.
Adding Newborns/Children
To add a child to DEERS, the sponsor must present the original birth & social security card (when available). If a stepchild is being added, the marriage certificate is also required.
Dependents Aged 21-23
Dependents aged 21-23 must be enrolled full-time in school to obtain an ID card. Provide a letter from their accredited school registrar's office confirming the enrollment status and an estimated graduation date.
Children Born Out of Wedlock
Female Sponsors: Provide child's birth cert. & SS card; Male Sponsors: Provice court order establishing paternity or State Voluntary Acknowledgement of Paternity Form,child's birth cert., SS card,& legal documents showing judicially determined paternity.
Enrolling a Dependent Ward
Sponsor should provide a DFAS letter of approval, court document placing the child in the members household, birth certificate, & Social Security card. Please note that all documents must be originals/certified copies and translated into English.
Enrolling a Parent Dependent
Upon enrollment, sponsor must provide a DFAS approval letter, sponsor's birth cert. & parent's birth cert., 2 forms of photo ID, & SS card. Documents must be originals/certified copies & translated into English.
Enrolling a Parent-in-Law Dependent
Follow instructions for Enrolling a Parent Dependent, except provide the Spouse's birth certificate and marriage certificate. All documents must be originals or certified copies and translated into English.
Civilian DoD Employees
New CAC Issuance: Provide PSC orders & 2 forms of valid photo ID; Renewal CAC Issuance: Provide extension letter, existing CAC & 1 form of valid photo ID; Personnel status must be pre-loaded into DEERS by employee's agency HR.
Civilian Local Hire Employees
This category includes MOD, NAF, LN-Direct Hire; New CAC Issuance: Provide DD 1172-2 & 2 forms of valid photo ID; Renewal CAC Issuance: Provide DD 1172-2, existing CAC, & 1 form of valid photo ID; Obtain the DD 1172-2 from employee's agency HR.
Contractor Employees
New CAC Issuance: Provide 2 forms of valid photo ID & DD 1172-2*; Renewal CAC Issuance: Provide existing CAC, 1 form of valid photo ID, & DD 1172-2*; *DD 1172-2 is NOT required if a contractor has been submitted & approved through CVS.
