The Office of the Staff Judge Advocate at Hurlburt Field provides legal assistance to active-duty, reserve component on orders or pending deployment and retired military members and their military identification card-eligible family members. This assistance includes wills, powers of attorney, and other civil legal matters such as landlord/tenant, domestic, and consumer affairs issues. All services are free of charge.
Power of Attorney and Notary Service Walk-ins:

Prior to visiting the office to have a Power of Attorney accomplished, please visit the Air Force Legal Assistance website at. At the home webpage, select "Legal Worksheets." You will then select "Create Power of Attorney." The program prompts for information, fill all information the webpage asks for. Once the form is completed, the website provides a ticket number. Bring your ticket number(s) in to accomplish Power of Attorney. This speeds up the process and ensures you have all information available at the time the Power of Attorney is drafted.
Wills/Living Wills/Healthcare Power of Attorney:

Prior to scheduling a will appointment, customers must complete the will worksheet at the Air Force Legal Assistance Website. Select "Legal Worksheets." The program prompts for information. Once the form is completed, the website provides a ticket number. Have your ticket number ready when you call to schedule your appointment.
Claims processing for property damage/loss due to shipment of household goods, hurricanes, vandalism, etc. are accomplished by the Air Force Claims Service Center located in Kettering, Ohio with 24-hour service. Personnel process their claims online at website provided. The base claims office accepts DD Forms 1840R (pink form) during duty hours in Room 234 within 70 days after delivery. The base legal office is available to assist claimants daily with scanning claims documents to the Claims Center.
Tax Center services are only provided during tax season, usually January through April. Please call our office for exact dates.
