DOCUMENTS
ALL DOCUMENTS MUST BE THE ORIGINAL OR CERTIFIED COPY. YOU WILL BE ASKED TO SHOW YOUR DOCUMENTS WHEN YOU SIGN IN. ALL ACTIONS MUST HAVE PROOF (I.E. PROMOTION,REDUCTION,BIRTHS,MARRIAGE,DIVORCE ETC.)
REPORT TO FRONT DESK UPON YOUR ARRIVAL
PLEASE REPORT TO THE FRONT DESK AT LEAST 10 MINUTES PRIOR TO YOUR APPOINTMENT TIME. SUPPORTING DOCUMENTATION WILL BE VERIFIED AT THE FRONT DESK
REQUIRED DOCUMENTS
All must have 2 forms of ID (1 must be a photo ID); CIV/DOD Employee-Recent SF50; Contractor must be in the Trusted Associate Sponsorship System (TASS) & have a valid AKO account; NAF and PX employees need a signed 1172-2. Retiring Military-DD 214 & order
RENEWAL CAC/RETIRED ID CARDS & CHILDREN 10 YRS OLD
ID Cards/CACS can be renewed 90 days prior to expiration date. Military can obtain their retired ID card 2 weeks prior to the actual retirement date. DOD civilians must have a current SF50. Children can receive an ID card at 10 YRS old
LOST/STOLEN ID (MILITARY)
All Soldiers must have ONE of the following to replace a lost/stolen CAC card: a statement/document as provided by the Service (e.g., counseling document) or letter/memorandum from the Soldier CDR/1SG (must be a wet signature or digitally signed).
LOST/STOLEN CAC CARD DOD/DA CIVILIANS
All must have one of the following: a report from the installation security office or letter/memorandum from the activity's security manager or your supervisor. Document must be prepared on the activity's letterhead, dated and signed with a wet signature.
LOST/STOLEN CAC CARD DOD/DA CONTRACTORS
Must have one of the following: report from the installation security office, letter/memorandum from the activity's security manager or sponsoring TASS Trusted Agent. Document must be on the activity's letterhead, dated & signed with a wet signature
LOST/STOLEN ID CARD DEPENDENT
Must have one of the following: MP report or statement/memorandum containing the date, place and circumstance of the loss or theft. Sponsor must be present or provide a DD Form 1172-2 or an original unexpired Power of Attorney to the family member.
LOST/STOLEN ID CARD RETIREE
Must provide one of the following: MP report, statement or memorandum describing the date, place, and circumstances of the loss or theft.
ISSUANCE OF DEPENDENT ID CARDS
Sponsor must be present, or dependent must have a DD Form 1172-2 signed by the sponsor and a verifying official or have sponsor's signature notarized. A Power of Attorney or current deployment orders may also be used. DD Form 1172-2 is valid for 90 days
INPUT/REMOVE FAMILY MEMBERS IN DEERS
Original or certified copy birth certificate, original or certified copy SSN Card(except for newborns), certified filed marriage certificate; For divorce: Final decree-filed & signed by the Judge; Female military-name change memo approved by MPD Chief
LEGAL CUSTODY WARD / WARD OF THE STATE
Court documents signed by Judge and filed, original or certified copy birth certificate, original or certified copy SSN card. Court document must state ward will be in the custody of sponsor for a minimum of 12 months.
CHILD OUT OF WEDLOCK (BIRTH CERTIFICATE REQUIRED)
Male sponsor must have a court order that establish paternity & the child's original or certified copy birth certificate or an approved dependency determination or a notarized voluntary acknowledgement of paternity. Female sponsor-birth certificate only
COLLEGE STUDENTS (AGE 21-22 YEARS)
Must have a letter from an accredited college/university, signed by the school registrar stating full time enrollment, working toward an associate degree or higher & anticipated graduation date. Sponsor must be present when issuing card or sign DD 1172-2
TRICARE FOR LIFE (MEDICAID/MEDICARE)
Bring in the medicaid/medicare card for verification or if card is lost, letter from Social Security Administration stating Medicare started/purchased. Enrollment can occur as early as 60 days prior to 65th birthday.

Have attempted 4 times in 4 visits to obtain presentable photo ID Workers there rude, condescending, uncaring, unfriendly- almost hostile- and lack ability to take acceptable photo IDs. Their manner, dress attire unprofessional. Four years ago, photo ID workers were professional, friendly. Those now there act and dress like lazy, insolent over-paid rude government workers bloated with too much authority Will travel to Ft. Benning or Maxwell to to get my needed presentable military photo ID.
